Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment tailored to residents in a skilled nursing setting
- Assess functional mobility, strength, balance, and fall risk to create meaningful plans of care
- Develop and implement individualized treatment plans with measurable goals that promote independence
- Collaborate with nurses, occupational therapists, speech therapists, and the interdisciplinary team to optimize resident outcomes
- Document progress and outcomes clearly and timely in the medical record
- Educate residents, families, and staff on best practices for mobility, safety, and exercise carryover
- Participate in care conferences and contribute clinical expertise to discharge planning and transitions
Qualifications
- Current Physical Therapist (PT) license in California or eligibility to obtain state licensure
- Active Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
- Experience in skilled nursing, geriatric, or post-acute care settings preferred
- Strong clinical reasoning, assessment, and documentation skills
- Comfort working as part of an interdisciplinary team and communicating with residents and families
- Ability to pass required background checks and onboarding screenings
Education
- Degree in Physical Therapy (DPT preferred, MPT or equivalent considered acceptable)
Knowledge & Skills
- Thorough knowledge of geriatric rehabilitation principles and skilled nursing care
- Proficiency in patient assessment, therapeutic exercise, transfer and gait training
- Skilled in fall risk assessment, mobility aids recommendation, and safe transfer techniques
- Clear and concise documentation and electronic medical record familiarity
- Effective communication and collaboration with clinical teams and families
- Time management and caseload prioritization in a busy post-acute environment
