Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment tailored to residents in a skilled nursing setting
• Assess functional mobility, strength, balance, and fall risk to create meaningful plans of care
• Develop and implement individualized treatment plans with measurable goals that promote independence
• Collaborate with nurses, occupational therapists, speech therapists, and the interdisciplinary team to optimize resident outcomes
• Document progress and outcomes clearly and timely in the medical record
• Educate residents, families, and staff on best practices for mobility, safety, and exercise carryover
• Participate in care conferences and contribute clinical expertise to discharge planning and transitions

Qualifications

• Current Physical Therapist (PT) license in California or eligibility to obtain state licensure
• Active Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
• Experience in skilled nursing, geriatric, or post-acute care settings preferred
• Strong clinical reasoning, assessment, and documentation skills
• Comfort working as part of an interdisciplinary team and communicating with residents and families
• Ability to pass required background checks and onboarding screenings

Education

• Degree in Physical Therapy (DPT preferred, MPT or equivalent considered acceptable)

Knowledge & Skills

• Thorough knowledge of geriatric rehabilitation principles and skilled nursing care
• Proficiency in patient assessment, therapeutic exercise, transfer and gait training
• Skilled in fall risk assessment, mobility aids recommendation, and safe transfer techniques
• Clear and concise documentation and electronic medical record familiarity
• Effective communication and collaboration with clinical teams and families
• Time management and caseload prioritization in a busy post-acute environment
