Candidate Pay Rate: $45 - $50 per hour

Roles & Responsibilities

• Evaluate residents' mobility, strength and functional status to build meaningful plans of care
• Implement therapeutic interventions and assist with restorative programs that improve quality of life
• Document patient progress clearly and promptly in the medical record to support team communication
• Collaborate closely with nurses, therapists and family members to ensure cohesive resident care
• Coach and educate residents and caregivers on safe transfers, home programs and fall prevention
• Monitor and report changes in condition to the supervising therapist and clinical team
• Assist with adaptive equipment selection and training to enhance independence

Qualifications

• Current Physical Therapist Assistant (PTA) license or certification required (California licensure preferred/required)
• BLS/CPR certification required or ability to obtain upon hire
• Experience working in skilled nursing, long-term care, or rehab settings preferred
• Strong interpersonal skills and a team-first mindset
• Ability to perform safe patient handling and follow infection control protocols

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Solid knowledge of therapeutic exercise, functional mobility and transfer techniques
• Comfortable with resident assessment, care planning and progress documentation
• Familiarity with assistive devices, gait training and adaptive equipment
• Proficient communication skills with residents, families and clinical teams
• Basic EMR navigation and accurate charting
• Good clinical judgment and the ability to prioritize caseloads
