Roles & Responsibilities
- Evaluate residents' mobility, strength and functional status to build meaningful plans of care
- Implement therapeutic interventions and assist with restorative programs that improve quality of life
- Document patient progress clearly and promptly in the medical record to support team communication
- Collaborate closely with nurses, therapists and family members to ensure cohesive resident care
- Coach and educate residents and caregivers on safe transfers, home programs and fall prevention
- Monitor and report changes in condition to the supervising therapist and clinical team
- Assist with adaptive equipment selection and training to enhance independence
Qualifications
- Current Physical Therapist Assistant (PTA) license or certification required (California licensure preferred/required)
- BLS/CPR certification required or ability to obtain upon hire
- Experience working in skilled nursing, long-term care, or rehab settings preferred
- Strong interpersonal skills and a team-first mindset
- Ability to perform safe patient handling and follow infection control protocols
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Solid knowledge of therapeutic exercise, functional mobility and transfer techniques
- Comfortable with resident assessment, care planning and progress documentation
- Familiarity with assistive devices, gait training and adaptive equipment
- Proficient communication skills with residents, families and clinical teams
- Basic EMR navigation and accurate charting
- Good clinical judgment and the ability to prioritize caseloads
