Roles & Responsibilities

• Evaluate residents and perform skilled PTA interventions to improve mobility and function
• Implement individualized treatment plans under the direction of the supervising PT
• Provide therapeutic exercise, gait training, transfers, and balance activities
• Collaborate with nursing and interdisciplinary teams to coordinate care
• Educate residents and families on safe mobility, home exercise programs, and fall prevention
• Document patient progress accurately and timely in the medical record
• Participate in discharge planning and goal-setting to support successful transitions
• Maintain a safe treatment environment and follow infection control protocols

Qualifications

• Licensed Physical Therapist Assistant (PTA) or eligible for licensure — California license preferred
• Current Basic Life Support (BLS/CPR) certification
• Experience in skilled nursing, long-term care, or rehab settings preferred
• Strong communication skills and a team-first attitude
• Ability to perform patient transfers and provide hands-on care safely
• Comfortable with routine documentation and using electronic medical records

Education

• Associate's degree from an accredited Physical Therapist Assistant program or equivalent

Knowledge & Skills

• Therapeutic exercise and functional mobility training
• Gait training, transfers, and assistive device use
• Basic therapeutic modalities and safe manual handling techniques
• Skilled observation and documentation of treatment outcomes
• Interdisciplinary collaboration and patient/family education
• Familiarity with skilled nursing facility workflows and regulations
