Roles & Responsibilities
- Evaluate residents and perform skilled PTA interventions to improve mobility and function
- Implement individualized treatment plans under the direction of the supervising PT
- Provide therapeutic exercise, gait training, transfers, and balance activities
- Collaborate with nursing and interdisciplinary teams to coordinate care
- Educate residents and families on safe mobility, home exercise programs, and fall prevention
- Document patient progress accurately and timely in the medical record
- Participate in discharge planning and goal-setting to support successful transitions
- Maintain a safe treatment environment and follow infection control protocols
Qualifications
- Licensed Physical Therapist Assistant (PTA) or eligible for licensure — California license preferred
- Current Basic Life Support (BLS/CPR) certification
- Experience in skilled nursing, long-term care, or rehab settings preferred
- Strong communication skills and a team-first attitude
- Ability to perform patient transfers and provide hands-on care safely
- Comfortable with routine documentation and using electronic medical records
Education
- Associate's degree from an accredited Physical Therapist Assistant program or equivalent
Knowledge & Skills
- Therapeutic exercise and functional mobility training
- Gait training, transfers, and assistive device use
- Basic therapeutic modalities and safe manual handling techniques
- Skilled observation and documentation of treatment outcomes
- Interdisciplinary collaboration and patient/family education
- Familiarity with skilled nursing facility workflows and regulations
