Roles & Responsibilities
- Deliver patient-centered therapeutic interventions to help residents regain mobility and independence
- Perform skilled interventions like gait training, balance work, transfers, and therapeutic exercise
- Collaborate with nursing, physicians, and other rehab staff to create and update individualized care plans
- Document progress clearly and timely in the facility charting system
- Educate patients and family members on safe mobility strategies and home programs
- Support safe transfers and fall-prevention strategies to improve overall resident safety
- Participate in team meetings and contribute to quality improvement efforts
Qualifications
- Licensed Physical Therapist Assistant (PTA) in California or eligible to obtain state licensure
- Graduated from an accredited PTA program
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Experience in skilled nursing or post-acute care preferred
- Strong clinical judgement, documentation skills, and a collaborative attitude
- Ability to pass background checks and meet facility credentialing requirements
Education
- Associate Degree from an accredited Physical Therapist Assistant program (required)
Knowledge & Skills
- Skilled in therapeutic exercise, manual techniques, and gait training
- Proficient with mobility aids, adaptive equipment, and safe transfer techniques
- Comfortable with goal-focused documentation and outcomes tracking
- Knowledge of infection control practices and resident safety protocols
- Excellent communication and teamwork skills with residents and interdisciplinary staff
- Ability to prioritize caseloads and manage time efficiently
