Roles & Responsibilities

• Deliver patient-centered therapeutic interventions to help residents regain mobility and independence
• Perform skilled interventions like gait training, balance work, transfers, and therapeutic exercise
• Collaborate with nursing, physicians, and other rehab staff to create and update individualized care plans
• Document progress clearly and timely in the facility charting system
• Educate patients and family members on safe mobility strategies and home programs
• Support safe transfers and fall-prevention strategies to improve overall resident safety
• Participate in team meetings and contribute to quality improvement efforts

Qualifications

• Licensed Physical Therapist Assistant (PTA) in California or eligible to obtain state licensure
• Graduated from an accredited PTA program
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Experience in skilled nursing or post-acute care preferred
• Strong clinical judgement, documentation skills, and a collaborative attitude
• Ability to pass background checks and meet facility credentialing requirements

Education

• Associate Degree from an accredited Physical Therapist Assistant program (required)

Knowledge & Skills

• Skilled in therapeutic exercise, manual techniques, and gait training
• Proficient with mobility aids, adaptive equipment, and safe transfer techniques
• Comfortable with goal-focused documentation and outcomes tracking
• Knowledge of infection control practices and resident safety protocols
• Excellent communication and teamwork skills with residents and interdisciplinary staff
• Ability to prioritize caseloads and manage time efficiently
