Roles & Responsibilities
- Assess, plan and deliver individualized physical therapy treatments for skilled nursing residents
- Collaborate with nursing and rehab teams to set measurable goals and track progress
- Document evaluations, treatment plans and outcomes accurately and on time
- Educate residents and families on safe mobility, fall prevention and home exercise programs
- Supervise and mentor therapy aides or assistants to extend patient care impact
- Participate in care conferences and contribute to interdisciplinary discharge planning
Qualifications
- Current, unrestricted California Physical Therapist license required
- Active BLS or CPR certification required; ACLS a plus
- Experience in skilled nursing or post-acute care preferred
- Strong communication skills and the ability to collaborate with clinical teams
- Comfortable with thorough documentation and goal-driven treatment planning
Education
- Doctor of Physical Therapy (DPT) preferred or Bachelor's in Physical Therapy with equivalent clinical experience
Knowledge & Skills
- Skilled in evaluation and treatment of mobility, balance and functional decline
- Proficient with therapeutic exercise, gait training and assistive device recommendations
- Knowledge of Medicare/Medicaid and skilled nursing documentation practices
- Ability to adapt treatments for medically complex and geriatric populations
- Good clinical reasoning, time management and patient education skills
