Roles & Responsibilities

• Assess patients’ functional mobility and develop individualized treatment plans
• Provide hands-on therapy, therapeutic exercise, and evidence-based interventions
• Collaborate with nurses, physicians, and rehab team members to coordinate care
• Document progress clearly and timely in the facility’s EMR
• Coach patients and families on safe transfer techniques and home exercise programs
• Participate in team meetings and contribute to discharge planning and goal setting

Qualifications

• Current, unrestricted California Physical Therapist license (required)
• Active BLS/CPR certification (or ability to obtain prior to start)
• Experience or interest in skilled nursing/long-term care settings (preferred)
• Strong communication and patient-centered care skills
• Ability to provide safe mobility and transfer training for medically complex patients
• Up-to-date immunizations and ability to pass background and health screenings

Education

• Doctor of Physical Therapy (DPT) or Master’s/Bachelor’s in Physical Therapy

Knowledge & Skills

• Skilled in evaluation and treatment of mobility, balance, strength, and ADLs
• Knowledge of rehab protocols for post-acute and medically complex patients
• Proficient with clinical documentation and interdisciplinary coordination
• Ability to educate patients, families, and caregivers clearly and compassionately
• Familiarity with safe patient handling and transfer techniques
