Roles & Responsibilities
- Assess patients’ functional mobility and develop individualized treatment plans
- Provide hands-on therapy, therapeutic exercise, and evidence-based interventions
- Collaborate with nurses, physicians, and rehab team members to coordinate care
- Document progress clearly and timely in the facility’s EMR
- Coach patients and families on safe transfer techniques and home exercise programs
- Participate in team meetings and contribute to discharge planning and goal setting
Qualifications
- Current, unrestricted California Physical Therapist license (required)
- Active BLS/CPR certification (or ability to obtain prior to start)
- Experience or interest in skilled nursing/long-term care settings (preferred)
- Strong communication and patient-centered care skills
- Ability to provide safe mobility and transfer training for medically complex patients
- Up-to-date immunizations and ability to pass background and health screenings
Education
- Doctor of Physical Therapy (DPT) or Master’s/Bachelor’s in Physical Therapy
Knowledge & Skills
- Skilled in evaluation and treatment of mobility, balance, strength, and ADLs
- Knowledge of rehab protocols for post-acute and medically complex patients
- Proficient with clinical documentation and interdisciplinary coordination
- Ability to educate patients, families, and caregivers clearly and compassionately
- Familiarity with safe patient handling and transfer techniques
