Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment tailored to each resident in a skilled nursing setting
- Develop and implement individualized care plans that promote mobility, independence, and safety
- Collaborate with nursing, occupational therapy, speech therapy, and physicians to coordinate comprehensive care
- Document progress and outcomes clearly and promptly to support continuity of care
- Educate residents and families on therapeutic goals, home exercise programs, and fall prevention strategies
- Monitor and adapt treatment approaches based on resident response and clinical best practices
Qualifications
- Current Physical Therapist license in the state of California (or ability to obtain quickly)
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program
- Experience in skilled nursing or geriatric rehabilitation preferred
- Valid Basic Life Support (BLS) or CPR certification
- Strong clinical reasoning, documentation skills, and the ability to work well within an interdisciplinary team
- Reliable, professional, and resident-centered approach to care
Education
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy from an accredited institution
Knowledge & Skills
- Proficiency in evaluation and treatment of mobility, balance, transfers, and gait training
- Knowledge of geriatrics, chronic disease management, and post-acute rehabilitation
- Skilled in creating measurable goals and progress notes consistent with facility standards
- Effective communication and patient/family education skills
- Familiarity with assistive devices, adaptive equipment, and safe transfer techniques
