Roles & Responsibilities

• Evaluate residents’ functional mobility and create individualized treatment plans that promote independence
• Deliver skilled therapeutic interventions including gait training, therapeutic exercise, balance and fall prevention
• Collaborate with nursing, OT, SLP and case management to coordinate comprehensive resident care
• Document progress notes and treatment outcomes clearly and timely in the medical record
• Educate residents and families on home exercise programs and safe mobility techniques
• Participate in care plan meetings and discharge planning to optimize outcomes and transitions

Qualifications

• Current Physical Therapist license in California (or compact/state authorization as applicable)
• Active BLS/CPR certification
• Prior experience in skilled nursing, rehabilitation, or post-acute care preferred
• Strong clinical assessment and manual therapy skills
• Excellent communication and teamwork skills; comfortable leading care discussions with families and staff

Education

• Doctor of Physical Therapy (DPT) or graduation from an accredited physical therapy program

Knowledge & Skills

• Skilled in patient assessment, treatment planning, and therapeutic exercise prescription
• Experience with mobility aids, transfer training and fall-risk mitigation
• Proficient in clinical documentation and use of electronic medical records
• Knowledge of post-acute rehabilitation protocols and regulatory requirements
• Ability to mentor rehab assistants and collaborate across disciplines
