Roles & Responsibilities

• Assess patients in the skilled nursing setting to establish functional goals and individualized plans of care
• Develop and implement evidence-based treatment plans that improve mobility, strength, balance, and independence
• Provide skilled therapeutic interventions, including gait training, therapeutic exercise, and assistive-device training
• Document progress clearly and timely in the medical record, communicating outcomes to the interdisciplinary team
• Collaborate with nurses, aides, therapists, and family members to ensure coordinated, patient-centered care
• Educate patients and caregivers on home exercise programs, fall prevention, and safe transfers
• Participate in care planning and discharge planning to optimize transitions and reduce readmissions
• Monitor and adjust treatment plans based on patient response and clinical best practices

Qualifications

• Current, active Physical Therapist license in California (required)
• Graduate degree in Physical Therapy (DPT preferred) or equivalent
• Basic Life Support (BLS) certification or willingness to obtain
• Experience in skilled nursing or post-acute care preferred
• Strong clinical reasoning, documentation skills, and teamwork mindset
• Ability to pass background checks and meet facility onboarding requirements
• Comfort with travel assignments and adapting to new teams and environments

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable with relevant experience

Knowledge & Skills

• Knowledge of geriatric rehabilitation principles and best practices
• Proficiency with therapeutic exercise, gait training, transfer training, and assistive devices
• Familiarity with wound care basics, fall risk assessment, and mobility aids
• Competence in clinical documentation and working within electronic medical records
• Excellent communication skills for working with patients, families, and multidisciplinary teams
• Time management and prioritization skills in a busy skilled nursing environment
