Roles & Responsibilities

• Assess patients' mobility, strength, balance and functional status to create individualized care plans
• Design and implement therapeutic exercise programs to improve independence and quality of life
• Collaborate with nursing, occupational therapy and rehab aides to coordinate patient-centered care
• Document patient progress clearly and timely, adjusting plans as patients improve or needs change
• Provide education and training to patients and families on safe mobility, fall prevention and home exercise
• Participate in interdisciplinary team meetings to advocate for patient goals and discharge planning

Qualifications

• Current California Physical Therapist license (required)
• Active CPR/BLS certification
• Experience working in skilled nursing or post-acute settings preferred
• Travel/contract therapy experience is a plus but not required
• Strong clinical reasoning and excellent communication skills
• Ability to manage a caseload and prioritize patient needs effectively

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy techniques and mobility training
• Knowledge of fall prevention strategies and adaptive equipment recommendations
• Familiarity with documentation standards for skilled nursing facilities
• Comfortable working as part of an interdisciplinary rehab team
• Good time management, organization and patient education abilities
