Roles & Responsibilities
- Assess patients' mobility, strength, balance and functional status to create individualized care plans
- Design and implement therapeutic exercise programs to improve independence and quality of life
- Collaborate with nursing, occupational therapy and rehab aides to coordinate patient-centered care
- Document patient progress clearly and timely, adjusting plans as patients improve or needs change
- Provide education and training to patients and families on safe mobility, fall prevention and home exercise
- Participate in interdisciplinary team meetings to advocate for patient goals and discharge planning
Qualifications
- Current California Physical Therapist license (required)
- Active CPR/BLS certification
- Experience working in skilled nursing or post-acute settings preferred
- Travel/contract therapy experience is a plus but not required
- Strong clinical reasoning and excellent communication skills
- Ability to manage a caseload and prioritize patient needs effectively
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent considered
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy techniques and mobility training
- Knowledge of fall prevention strategies and adaptive equipment recommendations
- Familiarity with documentation standards for skilled nursing facilities
- Comfortable working as part of an interdisciplinary rehab team
- Good time management, organization and patient education abilities
