Roles & Responsibilities

• Provide compassionate direct patient care and support with activities of daily living
• Monitor and record vital signs, intake/output, and changes in patient condition
• Document patient care accurately and timely in the facility charting system
• Communicate effectively with nurses and the care team to escalate concerns
• Assist with safe transfers, ambulation, and mobility to promote patient independence
• Ensure infection control standards and patient safety protocols are followed
• Support a positive patient experience through respectful, attentive interactions

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (NH or applicable VT credential)
• Local candidates only, able to commute to Lebanon, NH
• Comfortable working 12-hour night shifts and the alternating weekly schedule
• Ability to pass required background checks, TB screening, and immunization requirements
• Strong communication and teamwork skills; patient-focused attitude

Education

• High school diploma or equivalent
• Completion of an approved Nursing Assistant/LNA training program

Knowledge & Skills

• Hands-on patient care and ADL assistance
• Basic vital signs measurement and reporting
• Familiarity with infection prevention and safe patient handling
• Accurate documentation and charting practices
• Good interpersonal skills and the ability to work within a clinical team
