Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide skilled therapy to improve mobility, strength, balance, and functional independence
- Collaborate with the interdisciplinary rehab team to optimize patient outcomes
- Educate patients and caregivers on exercises, safety, and mobility techniques
- Maintain accurate and timely documentation to support continuity of care
Qualifications
- Active California Physical Therapist (PT) license required
- SNF or rehabilitation experience preferred
- Strong clinical judgment and excellent communication skills
- Ability to work independently while contributing effectively as part of a team
Knowledge & Skills
- Physical therapy assessment and treatment planning
- Skilled therapy provision focused on mobility and functional independence
- Patient evaluation and progress monitoring
- Clear communication with patients, families, and interdisciplinary teams
- Clinical documentation and rehab-focused care coordination
