Roles & Responsibilities
- Evaluate patients and create individualized physical therapy treatment plans that restore mobility and independence
- Deliver skilled therapy to improve strength, balance, and functional outcomes
- Collaborate with an interdisciplinary rehab team to coordinate care and optimize recovery
- Educate patients and caregivers on exercises, safety strategies, and mobility techniques to support long-term progress
- Maintain accurate, timely documentation to ensure high-quality care and continuity
Qualifications
- Active California Physical Therapist (PT) license
- SNF or rehabilitation experience preferred
- Strong clinical judgment demonstrated in patient care decisions
- Excellent communication skills for patient and team interactions
- Ability to work independently and as part of a collaborative team
