Roles & Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans that promote recovery and independence
• Deliver skilled therapy to improve mobility, strength, balance, and functional outcomes
• Collaborate with an interdisciplinary rehab team to create coordinated, patient-centered care plans
• Educate patients and caregivers on exercises, safety, and mobility techniques to support long-term progress
• Maintain accurate and timely documentation to ensure continuity of care and regulatory compliance

Qualifications

• Active California Physical Therapist (PT) license required
• SNF or rehabilitation experience preferred — bring your skilled nursing know-how
• Strong clinical judgment with the ability to assess and adapt treatment plans
• Excellent communication skills for working with patients, families, and the care team
• Comfortable working independently and as part of a collaborative team
