Roles & Responsibilities

• Assess patients’ physical abilities and develop individualized rehabilitation plans
• Deliver skilled therapeutic interventions to restore mobility, reduce pain, and improve function
• Document evaluations, progress notes, and discharge plans clearly and promptly
• Collaborate with nurses, physicians, and interdisciplinary team members to coordinate care
• Educate patients and families on home exercise programs and safe mobility strategies
• Monitor patient progress and adjust treatment plans to meet evolving clinical goals
• Contribute to a positive, safety-focused environment that supports patient dignity

Qualifications

• Licensed Physical Therapist (PT) in California or able to obtain state licensure promptly
• Current CPR/BLS certification (or willing to obtain upon hire)
• Experience in skilled nursing or post-acute rehabilitation preferred
• Strong documentation skills and familiarity with electronic medical record systems
• Excellent communication and teamwork skills to work with patients, families, and staff
• Ability to pass background check and drug screening as required by facility

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in physical therapy or related field accepted

Knowledge & Skills

• Clinical expertise in mobility training, therapeutic exercise, and balance/fall prevention
• Skilled in patient assessment, treatment planning, and outcome measurement
• Knowledge of skilled nursing regulations and post-acute care best practices
• Proficiency with documentation standards and rehab billing basics
• Ability to teach and motivate patients and caregivers effectively
• Time management and prioritization in a fast-paced clinical setting
