Roles & Responsibilities
- Assess patients' functional mobility and design individualized treatment plans that help them regain independence
- Deliver skilled therapeutic interventions—gait training, balance work, therapeutic exercise—with clarity and compassion
- Collaborate closely with nursing, physicians, and family members to coordinate care and optimize patient outcomes
- Document progress and outcomes accurately in the medical record to support continuity and quality of care
- Educate patients and caregivers on home programs, fall prevention, and safe transfers to promote lasting gains
- Participate in interdisciplinary care planning and discharge readiness to ensure smooth transitions
Qualifications
- Current California Physical Therapy license (required)
- Graduation from an accredited Physical Therapy program (DPT preferred)
- Current BLS/CPR certification
- Experience in skilled nursing, inpatient rehab, or post-acute care is highly valued
- Strong communication and teamwork skills—able to motivate patients and work with multidisciplinary teams
- Ability to meet facility-specific health requirements (vaccinations, TB test) and credentialing
Education
- Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable where applicable
- Active state licensure (California Physical Therapy license required)
Knowledge & Skills
- Skilled in mobility and functional assessments with an eye for practical, patient-centered solutions
- Proficient in therapeutic exercise prescription, gait training, balance and fall prevention strategies
- Familiarity with common post-acute conditions and adaptive equipment
- Comfortable using electronic medical records for documentation and care planning
- Strong patient education skills and the ability to train caregivers effectively
- Time management and prioritization to handle a busy caseload while maintaining quality
