Roles & Responsibilities

• Assess residents' functional abilities and design individualized occupational therapy treatment plans that promote independence
• Provide skilled therapeutic interventions focused on activities of daily living, adaptive techniques, and equipment training
• Collaborate with nursing, physical therapy, speech therapy, and the interdisciplinary care team to coordinate patient-centered care
• Document evaluations, progress notes, and discharge plans accurately and in a timely manner
• Educate residents and family members on strategies to maximize safety and participation at home and in the community
• Recommend and train on adaptive equipment, splints, and assistive devices to improve function
• Participate in care conferences, QA initiatives, and continuous improvement efforts that impact resident outcomes
• Mentor and support therapy aides and students when applicable, fostering a positive learning environment

Qualifications

• Active, unencumbered Occupational Therapist license (California preferred) or ability to obtain state licensure
• Entry-level to experienced OTs welcome; clinical experience in skilled nursing or long-term care is a strong plus
• Current BLS/CPR certification
• Strong documentation skills and familiarity with electronic medical records
• Excellent communication and teamwork skills with a patient-centered approach

Education

• Doctorate or Master of Occupational Therapy, or equivalent entry-level OT degree
• State licensure required

Knowledge & Skills

• Skilled in ADL assessments, adaptive techniques, and mobility training
• Knowledge of orthotics, splinting, and adaptive equipment recommendations
• Experience with cognitive and perceptual intervention strategies for older adults
• Familiarity with rehab documentation, treatment planning, and outcome measurement
• Ability to work collaboratively within an interdisciplinary team and lead therapy sessions confidently
