Roles & Responsibilities

Evaluate patients and create individualized physical therapy treatment plans that help restore function
Deliver skilled therapy to improve mobility, strength, balance, and independence
Collaborate with an interdisciplinary rehab team to drive optimal patient outcomes
Educate patients and caregivers on exercises, safety, and mobility strategies to support recovery at home
Maintain accurate, timely documentation to ensure continuity of care and regulatory compliance
Use clinical judgment to adapt interventions and escalate care when needed

Qualifications

Active California Physical Therapist (PT) license required
SNF or rehabilitation experience preferred
Strong clinical judgment and clear communication skills
Ability to work independently and as part of a collaborative team

Knowledge & Skills

Physical therapy assessment and patient evaluation
Individualized treatment planning and therapy provision
Skilled interventions to improve mobility, strength, and balance
Effective communication with patients, families, and interdisciplinary teams
Sound clinical reasoning and documentation practices
Teamwork and collaborative problem solving in a rehab setting
