Roles & Responsibilities
- Evaluate patients and create individualized physical therapy treatment plans that help restore function
- Deliver skilled therapy to improve mobility, strength, balance, and independence
- Collaborate with an interdisciplinary rehab team to drive optimal patient outcomes
- Educate patients and caregivers on exercises, safety, and mobility strategies to support recovery at home
- Maintain accurate, timely documentation to ensure continuity of care and regulatory compliance
- Use clinical judgment to adapt interventions and escalate care when needed
Qualifications
- Active California Physical Therapist (PT) license required
- SNF or rehabilitation experience preferred
- Strong clinical judgment and clear communication skills
- Ability to work independently and as part of a collaborative team

- Physical therapy assessment and patient evaluation
- Individualized treatment planning and therapy provision
- Skilled interventions to improve mobility, strength, and balance
- Effective communication with patients, families, and interdisciplinary teams
