Roles & Responsibilities

• Assist the Physical Therapist in implementing individualized treatment plans that improve patient mobility and independence
• Provide therapeutic exercises, gait training, and functional mobility interventions tailored to geriatric needs
• Monitor patient progress and document outcomes accurately and timely in the EMR
• Educate patients and caregivers on exercises, safety strategies, and home techniques to support recovery
• Ensure compliance with facility policies, state regulations, and ethical standards to maintain high-quality care
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes and continuity of care
• Maintain timely and accurate documentation to support clinical decision-making and reimbursement

Qualifications

• Active California PTA license (required) — must be ready to practice in-state
• Graduate of an accredited Physical Therapist Assistant (PTA) program
• Current CPR/BLS certification
• Previous Skilled Nursing Facility (SNF) experience preferred
• Strong clinical reasoning, communication, and documentation skills
• Able to work independently while thriving as part of a multidisciplinary team

Education

• Graduate of an accredited Physical Therapist Assistant (PTA) program
