Roles & Responsibilities
- Assist the Physical Therapist in implementing individualized treatment plans that improve patient mobility and independence
- Provide therapeutic exercises, gait training, and functional mobility interventions tailored to geriatric needs
- Monitor patient progress and document outcomes accurately and timely in the EMR
- Educate patients and caregivers on exercises, safety strategies, and home techniques to support recovery
- Ensure compliance with facility policies, state regulations, and ethical standards to maintain high-quality care
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes and continuity of care
- Maintain timely and accurate documentation to support clinical decision-making and reimbursement
Qualifications
- Active California PTA license (required) — must be ready to practice in-state
- Graduate of an accredited Physical Therapist Assistant (PTA) program
- Current CPR/BLS certification
- Previous Skilled Nursing Facility (SNF) experience preferred
- Strong clinical reasoning, communication, and documentation skills
- Able to work independently while thriving as part of a multidisciplinary team
Education
- Graduate of an accredited Physical Therapist Assistant (PTA) program

- Therapeutic exercise prescription and progression
- Gait training and mobility assistance techniques
- Functional mobility assessment and intervention
- Clinical documentation best practices and EMR proficiency
