Roles & Responsibilities

• Provide compassionate, hands-on care to residents by assisting with activities of daily living
• Monitor and record vital signs and report meaningful changes to the nursing team
• Support mobility and safe transfers, promoting independence whenever possible
• Assist with meal service and feeding while observing dietary needs and preferences
• Maintain accurate documentation and participate in shift handoffs to ensure continuity of care
• Follow infection control procedures and facility policies to keep residents safe
• Work collaboratively with nurses and interdisciplinary team members to meet resident goals

Qualifications

• Valid Licensed Nursing Assistant (LNA) or T-LNA credential required
• Active/state-appropriate LNA license or ability to obtain NH state authorization prior to start
• Current CPR/BLS certification required
• Willingness to complete facility background checks and immunizations as required
• Travel healthcare experience or long-term care experience is a plus but not strictly required
• Strong communication skills, empathy, and a team-first attitude

Education

• High school diploma or GED required
• Completion of an approved LNA training program and state certification

Knowledge & Skills

• Skilled in personal care, bathing, grooming, and toileting assistance
• Proficient with taking and documenting vital signs accurately
• Able to assist with safe resident transfers and mobility support
• Familiar with infection control practices and PPE use
• Comfortable with basic charting and handoff communication
• Patient, observant, and able to escalate concerns appropriately
• Teamwork-oriented and adaptable in a fast-paced care setting
