Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Perform timely vital signs, documentation, and reporting changes in condition to nursing staff
• Support safe mobility and transfers, using proper body mechanics and equipment
• Assist with meal service and ensure residents’ nutrition and hydration needs are met
• Maintain a clean, safe environment and follow infection control protocols
• Collaborate with the care team to implement individualized care plans
• Build trusting relationships with residents and families through respectful communication

Qualifications

• Current Licensed Nursing Assistant (LNA) or T-LNA credential required
• Ability to work the assigned mix of 12‑hour and 8‑hour day shifts
• Current CPR/BLS certification preferred (or willingness to obtain)
• Comfortable with direct patient care, documentation, and teamwork
• Professional attitude, strong communication skills, and reliability

Education

• High school diploma or equivalent; successful completion of an LNA training program

Knowledge & Skills

• Skilled in patient hygiene, ADLs, feeding assistance, and mobility support
• Ability to take and record vitals accurately and communicate findings
• Familiarity with basic infection control and safety practices
• Good documentation habits and clear verbal communication with the team
• Comfortable working in long-term care and rehab environments
