Roles & Responsibilities

• Perform in-person participant assessments to determine appropriate levels of care and ensure safety
• Conduct regular home visits to deliver wound care, IV hydration, lab draws, and nurse assessments
• Respond promptly to calls from PCPs, triage teams, and support staff and document all contacts
• Initiate emergency treatment when necessary and coordinate follow-up care
• Document thoroughly in the EMR, complete incident reports, and update orders as needed
• Provide education and support to participants, families, caregivers, and facility staff
• Collaborate with providers, DME suppliers, pharmacies, paramedics, and community partners
• Participate in quality improvement initiatives, staff meetings, and orientation programs

Qualifications

• Active RN license (Michigan) required
• Associate degree in Nursing required; Bachelor's degree in Nursing preferred
• Minimum of one year experience with frail, elderly, or long-term care populations
• Minimum of one year home care experience preferred
• Minimum of one year wound care experience preferred
• Current AHA CPR certification or willingness to obtain
• Reliable transportation, valid Michigan driver’s license, and car insurance
• Must be 18+ and willing to meet vaccination requirements (Influenza, MMR, Varicella, Tdap/Td, COVID-19, Hepatitis B)

Education

• Associate degree in Nursing (required)
• Bachelor's degree in Nursing (preferred)

Knowledge & Skills

• Strong patient assessment and full-body assessment skills
• Experience with wound care, IV hydration, and lab draws
• Proficient EMR documentation and accurate record-keeping
• Excellent triage, critical thinking, and emergency response skills
• Effective communication and ability to educate participants and families
• Ability to collaborate across interdisciplinary teams and community partners
• Comfort working independently in the field and managing variable workloads
