Roles & Responsibilities

Conduct thorough clinical assessments, screenings, diagnoses, and treatment planning for patients from birth to elders
Deliver brief therapy and continuing care to support families and individuals
Lead discharge planning for smooth transitions to ALF or other placements
Coordinate care with multidisciplinary teams and community partners for optimal outcomes
Provide vital client advocacy, skill-building, and assistance with complex issues like Power of Attorney
Maintain precise, compliant documentation to meet all standards and ensure quality care
Facilitate patient care conferences and offer support for chemical dependency and mental health needs

Qualifications

Masters Degree in Social Work, Psychology, Counseling or related field
Licensed as LCSW, LPC, LMFT or in process
Valid Alaska Driver's License or obtainable within 6 months
Must be 21 years of age
Clinical Competency required within 3 months

Education

Masters Degree in Social Work, Psychology, Counseling or related discipline from accredited institution

Knowledge & Skills

Expertise in discharge planning and interviewing techniques
Crisis intervention and chemical dependency support
Ethical and legal management practices
Case management with tact and independent judgment
Mental health referrals and Power of Attorney assistance
Accurate documentation per accreditation standards
