Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health therapy to patients in their own spaces
Develop personalized treatment plans that make real differences in mobility and independence
Collaborate with interdisciplinary teams to optimize patient outcomes
Educate families on care techniques for lasting impact

Qualifications

Active Physical Therapy license in good standing
BLS certification required
Experience in home health settings a plus
Epic experience preferred but not required

Education

Graduate of an accredited Physical Therapy program
State licensure as a Physical Therapist

Knowledge & Skills

Strong clinical assessment and treatment skills
Excellent patient communication and empathy
Familiarity with home health documentation
Epic system knowledge is a bonus
