Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions to optimize outcomes
Master complex discharge planning for safe patient returns home
Collaborate with multidisciplinary teams to navigate care pathways
Utilize Meditech and Midas systems to track and document progress
Advocate fiercely for patients ensuring they receive top-tier support
Drive efficient case management in a dynamic hospital setting

Qualifications

Active RN license in good standing
Minimum 1 year of case management experience
Proficiency in Midas and Meditech electronic medical records
Experience with complex discharge planning preferred
Strong clinical judgment and patient advocacy skills
BLS certification

Education

Associate or Bachelor's degree in Nursing from an accredited program

Knowledge & Skills

Expertise in RN case management practices
Hands-on experience with Midas and Meditech EMR systems
Skilled in complex discharge planning
Excellent communication and collaboration abilities
Keen attention to patient ratios and care coordination
