Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, 8-hour day shifts

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a daily caseload of 5-6 new patients and 3-4 follow-up visits
Lead meaningful goals-of-care discussions with patients and families
Educate patients on disease processes, advance directives, and POLSTs
Facilitate family meetings and provide compassionate end-of-life support
Review patient charts and assess individual needs across the hospital
Collaborate with physicians and the palliative care team during daily rounds

Qualifications

Active California RN license
BLS certification required
ELNEC certification preferred
2-5 years of recent palliative or hospice experience
Acute care experience required
ICU or oncology experience a plus

Knowledge & Skills

Strong clinical thinking and communication skills
Flexibility and excellent organizational abilities
Compassionate approach to serving patients and families
Cerner EMR experience
Ability to work independently across multiple hospital units
