Location: Carmichael, California

End Date: 09-22-2026

Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage daily patient lists and review charts to identify those needing palliative support
Conduct comprehensive patient assessments and develop individualized care plans
Lead meaningful goals of care discussions with patients and families
Provide education on disease processes, advance directives, and POLST documentation
Facilitate family meetings and offer compassionate end-of-life support
Participate actively in daily rounds with the interdisciplinary team
Coordinate care across multiple hospital units as the palliative team serves patients throughout the medical center

Qualifications

Active California RN license required
2-5 years of recent palliative care or hospice experience
BLS certification required
ELNEC certification preferred
Acute care experience strongly preferred
ICU or oncology experience considered a plus
Strong clinical thinking and communication skills essential

Knowledge & Skills

Palliative care screening and consultation processes
Goals of care conversations and family facilitation
Advance directive education and POLST completion
Cerner electronic medical record system
Strong organizational and time management abilities
Compassionate approach to serving patients and families
