Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using a provided tablet and HCHB EMR
Collaborate with an Intake RN and Manager while enjoying autonomy in the field

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years of experience if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of around 40 miles

Knowledge & Skills

Skilled in orthopedic and geriatric rehabilitation
Familiar with managing patients who have multiple comorbidities such as CHF and COPD
Proficient with EMR documentation using HomeCare HomeBase
Strong communication and time-management abilities for independent home visits
