Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy evaluations and treatments in patients' homes
Complete start of care, recertifications, and daily visits for diverse patient populations
Develop personalized care plans for patients with CHF, orthopedic conditions, and pneumonia
Collaborate with clinical managers and educators to ensure optimal patient outcomes
Document patient progress and communicate effectively with the healthcare team

Qualifications

Current Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Valid driver's license and reliable transportation
Strong communication and time management skills

Knowledge & Skills

Home health physical therapy practices
Patient assessment and care planning
Electronic documentation systems
Common diagnoses including CHF, orthopedic conditions, and pneumonia
Time management for field-based visits
