Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Conduct OASIS assessments and Start of Care visits for new patients
- Provide skilled physical therapy services across four rural counties
- Help patients recover from falls, fractures, and age-related mobility challenges
- Document visits and track time using facility tablet system
- Manage a manageable caseload of 4-6 patient visits daily
Qualifications
- Current Physical Therapist license in Indiana
- Experience with OASIS assessments and Start of Care documentation
- Comfortable working independently in rural settings
- Strong communication and time-management skills
