Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy care to patients in their homes
Conduct start of care assessments, recertifications, and resumption of care visits
Deliver 6.5 points of care per day, ensuring each patient receives personalized attention
Collaborate with a supportive clinical team including coordinators and directors
Complete safety evaluations and teaching visits when needed for hospice patients
Document visits thoroughly and maintain high standards of care

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
Valid driver's license and auto insurance required
Home Health experience and OASIS training preferred
First time travelers welcome to apply

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Comfortable working independently in home settings
Excellent communication and documentation abilities
Familiarity with OASIS documentation preferred
Ability to manage a caseload of 5-6 visits per day
