Roles & Responsibilities

Deliver compassionate diabetes education to patients and families, transforming lives one lesson at a time
Collaborate with multidisciplinary teams using the nursing process to create personalized care plans
Administer medications and treatments with precision and care
Document meticulously to uphold the highest standards
Champion patient safety and experience through critical thinking
Serve as a vital resource for staff on education programs and materials
Advocate fiercely for patients across clinical, psychosocial, and other needs

Qualifications

Licensed as an RN in Illinois and Wisconsin
CDCES certification required
Approved CPR certification within 90 days
Graduate of an accredited nursing program, BSN preferred

Education

Graduate of an accredited nursing program, BSN preferred

Knowledge & Skills

Expertise in diabetes management and education
Strong interdisciplinary collaboration skills
Critical thinking for patient safety and escalation
Patient advocacy across clinical and psychosocial domains
