Roles & Responsibilities

• Coordinate and plan safe, timely discharges by collaborating with internal and external care teams
• Conduct comprehensive biopsychosocial assessments and initial evaluations
• Develop and document individualized plans of care and needs assessments
• Arrange durable medical equipment, medical transportation and home services as needed
• Refer patients to rehab, long-term care, and community resources to ensure continuity
• Provide clear, concise documentation in EPIC to support care coordination and transitions
• Apply protective services knowledge when assessing guardianship, power of attorney, or vulnerability risks
• Support collaborative care efforts across ED, ICU, Med-Surg, L&D/Mother-Baby and NICU when floating

Qualifications

• Current Washington clinical license (LICSW, LSWAIC) or nursing license required; pending licenses accepted at submittal
• BLS certification required
• RN or MSW candidates accepted; facility will accept LMSW or LCSW with Master degree for social work roles
• Minimum of 1 year of acute care and/or care coordination/discharge planning experience
• EPIC charting experience required
• Comfort floating between units and campuses as needed
• Ability to work every other weekend and meet scheduling commitments; include all RTO at time of submittal

Education

• MSW (Master of Social Work) for social work roles
• Active RN licensure for nursing roles

Knowledge & Skills

• Care coordination and discharge planning expertise
• Biopsychosocial assessment and initial evaluation skills
• Strong documentation and communication skills in EPIC
• Knowledge of Medicaid, Medicare, medication assistance and community programs
• Familiarity with guardianship, protective services, power of attorney and advance directives
• Experience with high risk intervention and managing patients with communication barriers
• Collaborative approach to coordinate services with rehab, home health and long-term care
