Roles & Responsibilities

• Evaluate residents’ mobility, strength, balance and functional status to create person-centered plans of care
• Develop and implement evidence-based treatment plans to improve function and independence
• Provide skilled therapeutic interventions including therapeutic exercise, gait and balance training, and functional task practice
• Collaborate with nursing, therapy aides and interdisciplinary team members to coordinate care and achieve resident goals
• Document skilled services timely and accurately in the facility medical record
• Educate residents and families on home programs, safety strategies and ongoing recovery goals
• Support smooth transitions and continuity of care while splitting time between two facilities

Qualifications

• Licensed Physical Therapist (PT) or DPT; eligible candidates who can obtain TN licensure are welcome
• Doctor of Physical Therapy (DPT) degree preferred (new graduates encouraged to apply)
• Comfortable providing skilled therapy in a Skilled Nursing Facility (SNF) setting
• Strong communication and teamwork skills for working across two sites
• Current BLS/CPR certification (or willingness to obtain) and compliance with facility onboarding

Education

• Doctor of Physical Therapy (DPT) preferred; new grads welcome

Knowledge & Skills

• Skilled in evaluation and treatment planning for older adults and post-acute care patients
• Proficient with documentation and clinical charting in SNF environments
• Knowledge of safe transfer and fall-prevention strategies
• Ability to triage, prioritize caseloads and adapt treatment across two facilities
• Strong patient- and family-centered education skills
