Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Monitor progress and communicate changes to physicians while maintaining detailed clinical records
Collaborate with home health aides and caregivers to ensure comprehensive patient support
Educate families on therapy goals and techniques to promote continued progress between visits

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice
Ability to work independently while providing exceptional patient-centered care

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient evaluation including muscle strength, mobility, gait, and range of motion
Care plan development and goal setting for rehabilitation
Family and caregiver education strategies
Documentation and clinical reporting best practices
Knowledge of Medicare, Medi-Cal, and Title 22 regulations
