Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy care to patients in their homes across Tehama County
Conduct Start of Care visits for orthopedic patients when nursing services are not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using a tablet and HCHB EMR system
Collaborate with an intake RN and manager to coordinate care and support patient goals

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than two years of experience if they have Home Health background
Comfortable traveling within the service area and managing a daily mileage of around 40 miles

Knowledge & Skills

Skilled in home health physical therapy assessments and interventions
Familiar with HCHB HomeCare HomeBase EMR documentation
Strong communication and time-management abilities for independent field work
Experience with orthopedic, geriatric, CHF, and COPD patient populations
