Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across your assigned region
Help patients recover from orthopedic injuries, strokes, joint replacements, and spinal conditions
Complete OASIS documentation to ensure compliance with government requirements
Collaborate with PTAs, OTAs, and rehab aides to provide comprehensive care
Build meaningful relationships with patients while guiding them through their rehabilitation journey
Contribute to a multidisciplinary team focused on improving quality of life

Qualifications

Active Physical Therapist license in California
At least 1 year of experience working as a PT, OT, or Speech Therapist
BLS certification required
Experience with OASIS documentation is essential
Home health setting experience is preferred but not required
Strong communication and time-management skills

Knowledge & Skills

Proficiency with OASIS documentation
Cerner EMR experience
Understanding of home-based care best practices
Ability to work independently in patients' homes
Compassionate approach to patient-centered care
