Shift Type: Day shift, 36 hours per week, flexible schedule covering assigned territories with possible weekends and travel outside area

Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Deliver exceptional physical therapy to patients in their homes across assigned territories
Perform evaluations, OASIS assessments, and treatments to improve mobility and quality of life
Collaborate with healthcare teams to create personalized care plans
Document visits thoroughly while balancing travel and patient care
Adapt to diverse patient needs including those with COVID considerations

Qualifications

Current CA PT License
BLS certification
Valid Drivers License with auto insurance and registration
Willingness to travel within territories like Napa, Vallejo, Fairfield and beyond

Education

Physical Therapy degree from accredited program

Knowledge & Skills

Expertise in home health rehabilitation therapy
Proficiency with OASIS documentation and evaluations
Strong patient assessment and treatment skills
Comfortable with per-visit productivity and travel
