Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for geriatric patients with multiple comorbidities including CHF and COPD
Complete approximately 5 meaningful patient visits per day while building lasting relationships
Document care using HCHB HomeCare HomeBase EMR on your provided tablet
Collaborate with the intake RN and manager to ensure seamless patient care coordination

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong assessment and clinical skills for orthopedic and geriatric populations

Knowledge & Skills

Home Health Physical Therapy expertise
Proficiency with HCHB HomeCare HomeBase EMR system
Ability to work independently while providing exceptional patient-centered care
Strong communication skills for patient education and family collaboration
Comfortable with tablet-based documentation and mobile healthcare delivery
