Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits for orthopedic patients when nursing services are not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using a tablet provided for documentation
Collaborate with an intake RN and manager to coordinate care and support patient goals

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but candidates with less than two years will be considered if they have Home Health exposure
Comfortable traveling within the service area and managing a daily caseload of five visits

Knowledge & Skills

HomeCare HomeBase (HCHB) EMR experience
Strong assessment and care-planning skills for orthopedic and geriatric patients
Ability to work independently while staying connected with the care team
Reliable transportation and willingness to drive approximately 40 miles per day
