Roles & Responsibilities

• Deliver skilled physical therapy interventions to help patients regain function and independence
• Implement and document treatment plans and progress notes in a timely, accurate manner
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate patients and families on home programs, safety, and strategies to maximize recovery
• Assist with transfers, mobility training, therapeutic exercises, and restorative care
• Participate in goal-setting, discharge planning, and continuity of care activities
• Maintain compliance with facility policies, safety protocols, and documentation standards

Qualifications

• 1-3 years of relevant clinical experience (explicit requirement)
• Qualified as a Physical Therapist Assistant (PTA) — licensed or eligible to be licensed in Arizona (facility may require state licensure)
• Experience in SNF/long-term care or rehabilitation settings preferred
• Excellent interpersonal skills and ability to work collaboratively with a multidisciplinary team
• Ability to float between Prescott Village and Prescott Valley locations as needed

Knowledge & Skills

• Strong knowledge of physical therapy principles and rehabilitation techniques
• Experience with geriatric care and SNF-specific restorative programs
• Skilled in patient assessment, therapeutic exercise, gait training, and transfer techniques
• Solid documentation skills and familiarity with clinical charting best practices
• Comfortable working in a fast-moving environment and adapting to changing patient needs
