Roles & Responsibilities

• Provide skilled occupational therapy evaluations and evidence-based treatment for SNF residents
• Develop individualized treatment plans and measurable functional goals
• Document patient progress accurately and timely in accordance with facility and CRG policies
• Collaborate with the interdisciplinary team to support resident-centered care and safe discharges
• Manage caseload efficiently while meeting productivity expectations
• Maintain professional presence and adhere to facility dress code (black scrubs) and policies

Qualifications

• 1-3 years of occupational therapy experience (as noted in the job request)
• Licensed or eligible for licensure as an Occupational Therapist in New Mexico (state licensure required)
• Experience or comfort working in a skilled nursing facility (SNF) setting
• Strong assessment, treatment planning, and documentation skills
• Ability to be present on-site (therapist must be in the building to receive guaranteed hours)
• Willingness to follow Continuum Rehabilitation Group (CRG) policies and productivity expectations

Education

• Degree in Occupational Therapy (entry-level OTD or Master's level program typical for the profession) — specific minimum not listed in job note

• Skilled in SNF rehab interventions and functional recovery approaches
• Competent in patient assessment, ADL training, and adaptive equipment recommendations
