Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Deliver personalized occupational therapy services in patients' homes across assigned geographical regions
Collaborate with PT and Speech colleagues to provide comprehensive rehab care
Complete OASIS documentation to meet government-mandated requirements
Support patients recovering from ortho conditions, strokes, joint replacements, and spinal injuries
Partner with rehab aides and permanent staff to optimize patient outcomes
Build meaningful relationships while helping patients regain independence

Qualifications

Active Occupational Therapist license in California
BLS certification required
At least 1 year of experience working in PT/OT/Speech
Experience with OASIS documentation required
Home health setting experience preferred but not required

Knowledge & Skills

Strong documentation skills with OASIS forms
Ability to work independently in home-based care environments
Comfortable with Cerner EMR system
Flexible and adaptable to varying patient needs
Excellent communication and interpersonal skills
