Contract Type: Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, or 10:00 PM - 6:30 AM with cycle scheduling, every other weekend and holiday

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents in a 40-bed long-term care setting
Support residents managing conditions such as CHF, COPD, Alzheimer's, Parkinson's, and diabetes
Collaborate closely with the charge nurse and care team to ensure excellent outcomes
Administer medications with attention to safety and accuracy
Build meaningful relationships in a home-like environment that values strong teamwork

Qualifications

Active LPN/LVN license in Minnesota
Current BLS certification required
Prior long-term care experience preferred
Strong communication and teamwork skills

Knowledge & Skills

Familiarity with PointClickCare EMR system
Understanding of common long-term care diagnoses
Ability to work collaboratively in a small, supportive team
Comfort providing care with minimal IV involvement
