Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients with professionalism
Administer medications and treatments accurately and document per facility protocols
Monitor and report changes in patient condition to the care team promptly
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Maintain thorough and timely documentation in patient charts and electronic records
Support a positive resident experience through respectful communication and empathy

Qualifications

Active LPN license (Licensed Practical Nurse) — eligible to practice in Pennsylvania
Completion of an accredited practical nursing program
Current CPR/BLS certification (preferred or able to obtain)
Previous experience in a clinical or long-term care setting is a plus
Strong communication, organization, and teamwork skills

Education

Completion of an accredited Practical Nursing (LPN/LVN) program
High school diploma or equivalent

Knowledge & Skills

Medication administration and accurate charting
Basic assessment skills and vital signs monitoring
Familiarity with infection control and patient safety practices
Effective communication with patients, families, and multidisciplinary teams
Time management and the ability to prioritize patient needs
