Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents following care plans
Administer medications and treatments accurately and document thoroughly
Monitor resident conditions, report changes to the care team, and escalate concerns
Collaborate with nurses, CNAs, and interdisciplinary staff to deliver quality care
Support resident activities of daily living and promote dignity and independence
Maintain clear, timely clinical documentation and follow facility policies

Qualifications

Current, active LPN license (required)
Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Valid immunizations and ability to pass required health screenings
Background check and drug screen clearance as part of hiring process
Strong communication skills and a team-oriented mindset
Previous experience in long-term care or skilled nursing preferred

Education

LPN diploma or vocational nursing certificate
Current LPN licensure for Pennsylvania

Knowledge & Skills

Medication administration and documentation best practices
Working knowledge of common clinical assessments and vital sign monitoring
Skilled at resident-centered care and charting in electronic or paper systems
Ability to prioritize tasks in a fast-paced care environment
Good interpersonal skills for working with residents and families
