Roles & Responsibilities

Provide compassionate direct patient care to residents in a long term care setting
Administer medications and document administration accurately and promptly
Monitor and report changes in patient condition to the care team
Collaborate with nurses, therapists, and physicians to deliver coordinated care
Complete timely and thorough charting in the facility medical records
Support residents with activities of daily living while promoting dignity and independence

Qualifications

Current Massachusetts LPN license required
CPR/BLS certification required or must obtain upon hire
Previous experience in long term care or skilled nursing preferred
Ability to pass background check and required facility onboarding
Strong communication skills and a team-oriented mindset

Education

Diploma or Certificate in Practical Nursing (LPN) or equivalent
High school diploma or GED

Knowledge & Skills

Skilled in medication administration and documentation
Familiar with infection control and patient safety practices
Comfortable with wound care basics and routine nursing procedures
Good clinical assessment skills and ability to escalate concerns
Proficient in electronic charting or willingness to learn
Compassionate patient-centered care and strong interpersonal skills
