Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments safely and accurately
Monitor and document patient condition changes in the medical record
Collaborate with nursing staff and caregivers to support individualized care plans
Ensure infection control and safety protocols are followed at all times
Communicate clearly and kindly with patients, families, and the care team

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification (required or obtain upon hire)
Willingness to complete facility orientation and any required onboarding checks
Background check and immunizations per facility policy (TB, MMR, COVID vaccination status as required)
Excellent communication skills and a patient-centered approach

Education

Graduate of an accredited Practical Nursing (LPN) program; current LPN license required

Knowledge & Skills

Fundamental nursing assessments and clinical observation skills
Medication administration and documentation best practices
Basic wound care and routine nursing procedures
Comfortable with electronic medical records and charting
Strong teamwork, time management, and patient advocacy skills
