Roles & Responsibilities

• Provide compassionate direct patient care in accordance with facility policies and nursing standards
• Administer medications and treatments accurately and safely
• Monitor patient status and report changes to the interdisciplinary team promptly
• Maintain thorough, timely documentation of patient care and observations
• Collaborate with nurses, providers, and support staff to ensure continuity of care
• Support admissions, discharges, and care transitions with attention to detail
• Participate in shift handoffs and contribute to a positive, team-focused environment

Qualifications

• Active Licensed Practical Nurse (LPN) licensure required
• Willingness to commit to every-weekend coverage as part of the rotation
• Ability to provide proof of required credentials and complete facility onboarding
• Reliable personal transportation required for this assignment
• Flexible, professional attitude and ability to work in a fast-paced clinical setting
• Adherence to facility RTO policy; all RTO must be approved prior to contract approval

Education

• Graduate of an accredited practical nursing program and LPN certification

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Strong patient assessment and observation abilities
• Clear, concise clinical documentation and charting
• Effective communication and teamwork with multidisciplinary staff
• Time management and prioritization in a busy unit
• Comfortable with electronic health record systems and standard clinical workflows
