Roles & Responsibilities
- Provide compassionate direct patient care and monitor patient status throughout your shift
- Administer medications and treatments accurately and safely per physician orders
- Document assessments, interventions, and patient responses in the medical record
- Collaborate with RNs, providers, and support staff to develop and update care plans
- Educate patients and families on care plans, medications, and discharge instructions
- Assist with admissions, transfers, and safe patient handoffs to ensure continuity
Qualifications
- Current Licensed Practical Nurse (LPN) license (state licensure required)
- CPR/BLS certification (or willingness to obtain)
- Prior experience in long-term care or similar clinical setting preferred
- Strong clinical assessment, medication administration, and documentation skills
- Clear background check and ability to meet facility onboarding requirements
Education
- Vocational Nursing diploma or equivalent LPN training program
Knowledge & Skills
- Solid knowledge of medication administration and basic nursing procedures
- Comfortable with charting and electronic medical records
- Strong communication and teamwork skills
- Ability to prioritize tasks and respond to changing patient needs
- Basic wound care and infection control practices
